Download and print a copy of the Post-Test and the Answer Sheet/Evaluation Form
Answer all test and evaluation questions; participants must correctly answer at least 70% of the test questions to receive credit
Mail or fax the Answer Sheet/Evaluation Form to:

No grading is done until the CME fee is received by FMA staff

Once the answer sheet is graded and a score of at least 70 percent is achieved, a certificate of credit will be mailed to you. Retain a copy of your certificate for your records.
